家政服务居间协议书
雇主（以下简称甲方）：家政服务员（以下简称乙方）：中介服务公司（以下简称丙方）：经丙方居间介绍，由乙方为甲方提供（育儿嫂、月嫂、家政保姆）家政服务，现就具体事宜约定如下：第一条 服务地点： 第二条 服务方式与期限1、丙方为甲、乙双方提供居间服务期限：2、乙方自 年 月 日起为甲方提供家政服务，具体工作时间为早 晚 3、乙方休息日： 天/月第三条 服务费用1、甲方支付乙方工资：人民币 元/月2、甲方与本居间服务期内支付丙方中介服务费：人民币 元（收费标准：家政服务首月工资的 ）3、乙方于本居间服务期内支付丙方中介服务费用：人民币 元（收费标准：家政服务首月工资的 ）4、甲乙双方委托丙方代收、代付首月工资，签订本协议时，甲方即向丙方支付首月工资的20%，七天试用期满后支付剩余首月工资。首月工资的作用及使用顺序如下：（1）扣除乙方应向丙方支付的中介服务费；（2）乙方首月工作满一个月或者未满一个月下岗，丙方代甲方支付相应的工资，并将余款返还甲方。其它月份的工资由甲方直接支付乙方。第四条 甲方权利义务1、甲方有权要求乙方重新进行体检，费用由甲方承担，如检验不合格则由乙方自行承担；2、甲方在接受丙方服务期内，如自行解除与乙方雇用关系，可以合理要求丙方另行介绍其它家政服务人员，并根据本协议第三条规定的收费标准向丙方支付相应的服务费差额，家政服务员的工资以甲方、丙方和家政服务员重新签订的《家政服务居间协议书》后，本协议自动解除；3、如果甲方在接受丙方服务期内提出不需要丙方介绍家政服务员的要求，所交服务费概不退换（因所交服务费为居间费用，不因服务期长短而增减，已经支付，甲方即不得要求返还）；4、甲方更换乙方不得要求更改协议规定的服务内容及服务方式，如需更改应另行签订协议书，并向丙方支付相应的服务费；5、乙方如在工作期间内超期休息，甲方有权扣除相应工资或顺延支付工资日期。如果是甲方的原因造成乙方不能正常工作，甲方应与乙方协商串休或正常支付工资；
